Qualified contractors who are interested in working with HiFive must complete the form and must be able to provide the following information, if requested:
Company Information
- Company license numbers
- List of recently completed projects inclusive of values
- Three references (please include company name and person of contact)

Essential Documents
- W-9 or country specific tax form equivalent
- Safety Program
- Insurance information – Current sample certificate and additional insured endorsement forms
